PREPARING FOR YOUR FIRST VISIT

Easy Online Forms

After scheduling your first appointment, you’ll receive a link to complete your new patient forms electronically.
We ask that forms be completed as soon as possible—at least 3 days before your visit—so our team can review them in advance.

Transfer of Records

Please contact your previous dental office and request that your clinical records and X-rays be emailed to:

If records are not received before your appointment, additional X-rays may be needed and may not be covered by insurance.

Antibiotic Premedication

If you have been told to premedicate with antibiotics prior to dental work, please take them as prescribed before your appointment. If you are not sure if you need antibiotics, please contact your doctor prior to your visit. Guidelines for premedicating for dental appointments have changed over the years, so please check with your doctor to make sure that they still recommend premedicating for your condition.
